Self enrollment is available for Torrance Memorial patients who have had a previous visit, are age 18 or older and don't currently have access to MyTorranceMemorial. MyTorranceMemorial is an online portal where you can securely view information about your health at any time. Complete the form below to start the self enrollment process.

If you manage the health of a patient, talk to a registration team member at their next visit to receive a personal invitation to MyTorranceMemorial. Self enrollment is not currently available for caregivers and guardians.
